Police said he remained in a serious but stable condition.
His passenger, a 48-year-old Peregian Beach woman, suffered serious non-life-threatening injuries and was also taken to hospital.
The Volkswagen driver, a 34-year-old Noosa Heads man, suffered serious injuries and was taken to hospital.
The Subaru driver, a 38-year-old Tewantin man, was assessed by paramedics at the scene and was not physically injured.
The Sunshine Motorway was closed in both directions while investigators examined the scene and reopened about 10.30am.
